PDI Update to Physicians – Feb. 17, 2010

Instructions to submit your VPL, Deposit Information and Validated Patient List Process forms to Alberta Health and Wellness are now available.

 Instructions – Validated Patient List Submission Information

Below are the two forms that must be submitted with your VPL in order for payment to be processed. These forms must be completed and received, along with your VPL, by Alberta Health and Wellness no later than March 15, 2010. The submissions may be made in advance of this date.

These two forms must be completed by each participating physician.

 Deposit Information Form (Enables Alberta Health and Wellness to process physician payments)
 Validated Patient List Process Form (Details how the physician intends to complete Level 2 Validation)

These forms can be submitted in one of the following ways:

• Scan and submit completed forms electronically along with your re-encrypted VPL (originals can be retained by the clinic/physician) to: health.pdi.program@gov.ab.ca; or

• Fax the forms to Alberta Health and Wellness attention: PDI Coordinator and include the “L” number on the cover sheet at 780.422.5208 (originals can be retained by the clinic/physician). Clinics need to take special care when faxing these documents, as they contain sensitive information i.e. Practitioner Identifier; or

• Forward by courier to Alberta Health and Wellness with the re-encrypted VPL CD if the clinic does not have easy access to internet/email; or
 
• Mail the forms to:

          Attention: PDI Coordinator
