Create a User Account 

 
To request a new user account please provide all the information on the form below. Your account information will be used to manage website user accounts and for website communications. Website administrators at PCI (and website administrators at PCNs that develop PCN template websites) will have access to the list of user accounts on the PCI website (names and e-mail addresses).

 Request a New PCI Website User Account

Complete the form below to request a PCI Website User Account. All fields are mandatory. The contact information will be used to verify your request. Your user name will be firstname.lastname (not case sensitive). You will receive a confirmation e-mail when your account has been activated.
