PCNs in Alberta 

 

Today, there are 40 PCNs operating throughout Alberta with more in development. More than 2500 family physicians in Alberta are working in PCNs.

Every PCN is unique because it is developed locally by area family physicians in cooperation with the local health region and other health professionals. Health professionals may include, among other regulated health professionals, nurses, nurse practitioners, social workers, pharmacists, mental health workers, exercise specialists or dietitians. This grassroots approach allows – and encourages – the PCN to focus on the needs of the local patient population.
