PCNs in Alberta 

 

Today, Alberta has 32 Primary Care Networks (PCNs) and another 11 in development. More than 60 per cent of all family physicians in the province are part of PCNs. The goal is to have 80 per cent of all Albertans receiving care from PCN teams by 2011.

Every PCN is unique because it is developed locally by area family physicians in cooperation with the local health region and other health professionals such as nurses and dietitians. This grassroots approach allows – and encourages – the PCN to focus on the needs of the local patient population.
