Submission Requirements 

 
The project plan is ready to be submitted when:
  • The timeline and detailed budget worksheets are completed.
  • The planning table on the Summary Budget worksheet in the Excel template attached to this information package is completely filled in. Note that the column headings and row labels on the Summary Budget worksheet of the template may not be altered as these numbers will be rolled up provincially.
  • The first row only of the reporting section on the Summary Budget worksheet in the Excel template attached to this information sheet is filled in. This is the financial report required to report the expenditures of the project that were funded by the initial $30,000 advanced to the PCN upon approval of the Letter of Intent. Please record the date of the reported data.
Submit the PCN’s project plan in hard copy, signed by the physician lead and AHS representative to:
