The Service Plan is developed using Section 3 (Priority Initiatives) and Section 4 (Financial Plan) of the Business Planning template. It is submitted as a draft for approval, along with a Budget Model (see Budgeting Section) to ensure that developing PCN’s are on the right track before they develop their full business plan. Using the Business Plan Template for the Service Plan Before taking Sections 3 and 4 from the Business Plan template, download the most current version from the Primary Care Initiative website. Before submitting the PCN’s Service Plan, ensure that the most current version of the template is being used. PCNs are responsible for ensuring that they are using the proper templates throughout the entire business planning process. Before opening the template, check the macro security settings on your computer. The macro security on the receiving computer must be set to “medium” for the macros to work. To view or change the macro security setting, go Tools>Macro>Security and select “medium,” then close and reopen Word for the setting to take effect. When prompted, "enable" macros. Instructions for filling out the template are located within the template document. Delete the insturctions when you are finished with them. Submitting the Service Plan Submit one hard copy and one electronic copy of the Service Plan for review. Be sure to include one hard copy and one electronic copy of the Budget Model (see Budgeting Section). Using the Budget Model Before submitting the PCN’s Business Plan, ensure that the budget model for the PCN complies with the instructions provided on the most current version of the Budget Model template. PCNs are responsible for ensuring that they are adhering to current policies at the time of Business Plan submission. Submitting the Draft Budget Model One hard copy and one electronic copy of the Budget Model are to be submitted with the Service Plan and with each submission of the draft Business Plan. Once the Budget Model has been submitted to the PCI Program Management Office, successive submissions must detail the changes from the previous submission. Submitting the Final Budget Model When submitting the final Business Plan for Primary Care Initiative Committee consideration, submit two hard copies of the Budget Model, along with an electronic copy.
