Transition to Implementation of a PCN 

 

Following the approval by PCIC of the PCN's three year Business Plan, the PCN needs to complete some outstanding documentation requirements directly with the PCI Program Management Office before approval shall be given to initiate the PCN and a per capita prostpective payment be issued. These requirements must be completed 20 days prior to the beginning of the month in which the PCN wishes to being operations. The transitional requirements are outlined (Note: an * indicates that the specific document is to be submitted directly to AHW instead of the PCI Program Management Office) :

  • Legal Joint Venture Agreement and all associated Schedules appended (see Legal Agreements)
    • Legal Joint Venture Agreement signed by NonProfit Corporation (NPC) physician lead and AHS lead
    • Individual Letters of Participation signed by each physician and NPC physician lead
    • Complete typed list of name and address for all participating physicians
  • NPC Certificate of Incorporation
  • NPC Articles of Association for the physician NPC (signed by the physician NPC leaders and witnessed)
  • NPC Memorandum of Association (signed by the physician NPC leaders and witnessed)
  • Assignment of Mark (signed by Physician lead and AHS lead)
  • Response letter from Office of Information and Privacy Commissioner (OIPC) to AHS and PCN regarding the status of Privacy Impact Assessment
  • Registration of Providers * (submit to AHW Primary Care Registration Coordinator)
  • Submitting Other Health Care Provider Encounters * (Submit to AHW Primary Care Registration Coordinator)
