Service Delivery Planning 

 

Each Primary Care Network (PCN) is responsible for the delivery of five key provincial objectives. (Schedule G to the Master Agreement). Priorities are developed according to the local needs of the population referred to as a Priority Initiatives. Each initiative is comprised of program elements which deliver specific service responsibilities directly related to the provision of primary care services to the patient population.

The Service Plans by which these service responsibilities are delivered vary as much as the composition of the PCNs which range from remote rural locations of less than 5 physicians to vast urban PCNs of over 200 physicians.
