PCN Website Template Toolkit
The PCN Website Template program is intended to standardize the visioning and creation of PCN websites using a website template supplied and hosted by PCI. My Health. My Primary Care Network. A Health Management Tool
The Primary Care Initiative has developed an online health information kit designed to help patients manage their health in partnership with their doctor and other health professionals. The kit allows people to keep a record of their health and see the changes over time. It also provides a place to collect health-related resources to help Albertans live healthier lives.

Team Development in Primary Care Networks (binder)
To help primary care networks build the teams they need to sustain practice change, Team Development in Primary Care Networks was developed as a resource guide and facilitation toolkit. It is targeted specifically to the individuals within PCNs who are designated as team facilitators: the leaders dedicated to helping PCNs achieve their multi-disciplinary team potential. Team Development in Primary Care Networks is an important starting point for individuals whose role is to lead the transformation of their PCN and its practitioners/staff into a highly functioning, multi-disciplinary team.
